2017-18 Kindergarten registration

Kindergarten registration opens today (Feb. 1st) to all residents of Upper Sandusky Schools.  Children who will be 5 years old on or before August 1, 2017 are required by law to register and be screened.

REGISTER YOUR CHILD BY ONE OF THE FOLLOWING MEANS:
  1. Click the following to register online --- 2017-18 Kindergarten Registration
    • You will then receive a follow-up phone call from the school within 48 hours to set up your appointment time.

  2. Call Brenda Haubert at the Superintendent's Office at 419-294-2306 between 8:30am-3:30pm.  
    Please have the following information ready:
    • Child's name, address, and phone number
    • Date of birth
After requesting an appointment, parents/guardians will receive a letter notifying them of their appointment time. The last day to register is March 31, 2017

INFORMATION THAT NEEDS TO BE BROUGHT TO YOUR SCHEDULED APPOINTMENT:
  • Birth certificate or other legal proof of age
  • Social Security card
  • Custody papers (if applicable)
  • Immunizations (shot records):  DTap - 5 doses, Polio - 4 doses, MMR - 2 doses given after age 1,
    Hepatitis B - 3 doses, Varicella (chickenpox) - 2 doses
During the screening your child will have an opportunity to meet the kindergarten teachers as well as participate in screening for vision, hearing, general readiness and speech and language.

The Wyandot County Health Department will be giving immunizations the day of the screening.

This is the only screening that will be held.  You must register for this screening if you want your child to attend Kindergarten in the 2017-2018 school year.
